Information we hold on you:
LLR PCL mainly commissions healthcare services and therefore do not hold any patient information to deliver healthcare. However, may have some information in regard to complaints, significant events and suggestions. In addition, this LLR PCL also provides the Referral Support Service (RSS) where healthcare providers screen referrals and direct to the most appropriate clinical setting. In addition to the above we hold staff information. This information will be held securely and is set out below why we need to collect this information.
Who we share your information with
As an organisation that provides services and contracts services, we do not handle all your information ourselves, so we need to delegate this responsibility to others within the organisation and also with our contracted services.
Once you have seen the care provider through one of our services, they will normally send details of the care they have provided you with to your GP practice, and also some information may be passed to us as the contractor, or service provider, which is mostly anonymised.
Your consent to this sharing of data, lays with your registered practice and with those others outside the practice is assumed and is allowed by the Law; however, we will gladly discuss this with you in more detail if you would like to know more.
The organisation and service provider staff (clinicians, administration staff) only access the information they need to allow them to perform their function and fulfil their roles.
We are required by Articles in the General Data Protection Regulations to provide you with the information in the following 9 subsections about the personal and special category data we hold for employment reasons

In addition to this, records will be destroyed completely after 7 years which the individuals employment ended. In certain circumstances and to comply with the law we may keep this longer. |
